Our consulting room is upstairs on the first floor, so if you have any mobility difficulties please contact us before completing this form so we can discuss your options.

Please let us know at the time of booking your appointment if there is a particular optometrist you would like to see and we will do all we can to book your appointment with your chosen optometrist.

If you have any sudden changes to your vision or any eye emergencies and we are not available, please contact your GP, phone 111 or attend the Eye Casualty Department at the New Claremont Wing of the RVI.
